Certificates of Insurance
To request a certificate of insurance or proof of "self-insurance"
coverage, send a written request to the Benefits and Risk Management
Department, including a copy of the original document (contract,
letter) requesting the information. The written request should provide
basic information as to the activity, services, or event to be covered,
including the dates and to whom the certificate should be issued.

for Driver's License Record (69 KB) New Form as of Aug-2016 - Complete this form for any district employee who rents,
leases or drives on district business. Send this completed form
to the Benefits and Risk Management Department.
Vehicle Accident Report Form (8 KB) - Complete this form to report an accident involving a district-owned vehicle. Send this completed form to the Benefits and Risk Management Department.
Loss Report Complete this form and submit it online. It will then transmit to the benefits department for processing.
