New Hires

Becoming a Plano ISD Employee

• Items to bring to your New Hire Meeting
• Information for teachers with out of state teaching certificates

Completed Forms to Bring to the New Hire Meeting

• I-9 - Complete to the signature and date line.
• W - 4 - Complete the lower portion of the form with the total number of allowances you are claiming.  Do not cut the form.
• Social Security Form  - Employee ID# is the social security number.
• Substitute Resignation Form - Complete if you are an active substitute for Plano ISD.
• Professional Development Hours Agreement  - professionals only
• Required Staff Development - professionals only
• Authorization Agreement for Automatic Deposit    - (optional) If completed, a voided check for each account must be attached.
• Employee Information Sheet - Two copies of this form are required.
• HR Policy Guide Acknowledgment
• Discipline Guidelines for Support Staff Personnel - paraprofessionals only
  
  

If you are unable to print and complete the above forms before the new hire meeting, forms will be available at your scheduled appointment time.

• Alternative Certification Payroll Deduction Form (if applicable)
